Home care is a service provided mostly in the home. Home care service is provided to people based on needs identified through client assessment. The belief of home care is to help people keep their independance in their own home and allow them to be close to their loved ones for as long as possible.

What Services are
a part of Home Care?


The services that make up home care are:


Client Assessment


This is carried out by:
- Talking with you and your family
- Talking your doctor and others who've cared for you
- Reviewing your health history
- A physical Check up


Personal Care


Help with activities such as bathing, foot care and getting dressed

Case Management


The next step after assessment. This is to make sure the plan for your care is right and is provided by the right care giver at the right time


Home Care Nursing


-Nursing care that is provided in your home
- teaching you and your family to help you care for yourself
- supervising workers who provide you with personal care

In-Home Respite


Caring for you when it is not safe for you to be alone while your family who usually cares for you has a rest


Home Support


Help with light housekeeping, laundry and meal preparation

Who Benefits from Home Care Services?


Persons of any ages who have assessed need who:
- Have been discharged from the hospital - Have an illness or disease needing follow up care
- Are unable to live alone while waiting for long time care
- Have a disability and are unable to live on their own
- Need nursing care in the home
- Choose to live at home instead of a long time care facility as long as it's safe, affordable and services are available.

How are people referred?


You may refer yourself or you may be referred by others such as family members, friends, neighbors and hospitals.


Who provides these services?


Home care is mainly provided by registered nurses, licensed practical nurses and certified home care aids/ personal care workers who are supported by registered nurses/licensed practical nurses.
